• Immunization Records (or Exemption Form)

• Proof of Residency (Driver's License, Utility Bill within the past 60 days, Lease, etc.)

• Most recent School Transcript(s)

• Learner's completed Learner-Driven Starter Kit

• Guardian's completed Learner-Driven Starter Kit
